If you have an urgent medical need after hours and need to talk to one of our pediatricians after our regular hours, please call our office at (718) 868-4808. A doctor is on call 24 hours a day. Somebody will return your call promptly.
Medical Emergencies
In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Total Family Care of the Five Towns and Far Rockaway, and they will contact us.
Billing Department
At Total Family Care of the Five Towns and Far Rockaway, we accept most major commercial insurance plans, New York State Medicaid, and many NYS Medicaid managed care plans. Please contact your insurance company, or you may call our office at (718) 868-4808 to find out if we accept your insurance plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you must pay at the time of your visit. We accept cash, checks, Visa, MasterCard, American Express, and Discover for your convenience. Referrals
If one of our doctors suggests a specialist evaluation or diagnostic testing requiring a referral, we will provide this for you as soon as possible. Some referrals require insurance preauthorization and cannot be processed immediately. Insurance guidelines prohibit us from backdating referrals. We need three business days for the processing of routine referrals.
Hospital Affiliations
The pediatricians at Total Family Care of the Five Towns and Far Rockaway are affiliated with and have privileges to admit patients to North Shore – Long Island Jewish Medical Center.
Prescription Requests
Prescriptions and refills for Total Family Care of the Five Towns and Far Rockaway patients are routinely issued during our office hours. You can request by calling our office at (718) 868-4808.It is helpful to anticipate your refill needs to avoid interrupting your medication therapy.
